Privacy Policy

How Companian collects, uses and protects information submitted through companian.health, and the choices you have.

This policy explains what information Companian Inc. (“Companian”, “we”) collects through companian.health, how we use it and the choices you have. Companian is headquartered in Miami, Florida, and works with physician practices across the United States.

This website is for physician practices

Our website forms are for practice owners, managers and clinicians who want to learn about our care management services. Please do not enter patient health information in any website form. This website does not collect, store or transmit protected health information (PHI). Patient information for the care management programs we run with practices is handled separately, in secure systems, and never through this website.

Information you give us

When you request an intro or a practice review, we collect what you enter in the form:

  • your name, email address and phone number;
  • your role and practice name;
  • practice details: specialty, state, number of providers, approximate number of Medicare patients, current care management status, preferred start date and, if you choose, your EHR system.

When you book a call, our scheduling tool records the time you pick and the contact details you confirm. If you email us, we keep that correspondence.

Information collected automatically

When you visit the site, we and our service providers collect:

  • pages viewed, time on page, scrolls, clicks on links and buttons, and use of the revenue calculator;
  • the page you first arrived on and the site that referred you;
  • campaign details in the link you followed, such as UTM tags and ad click identifiers from Google or Meta;
  • device and browser information and an approximate location based on your IP address.

We use cookies and similar browser storage to do this. Some remember your first and latest visit so that, if you fill in a form, we can see which page or campaign brought you to us.

How we use information

  • To reply to your request, schedule your call and prepare for it.
  • To decide whether our services fit your practice and to follow up about them.
  • To understand how the site is used and to improve it.
  • To measure which advertising and campaigns bring practices to us.
  • To keep the site secure and to meet legal obligations.

We do not sell your personal information for money. We use advertising and analytics tools, described below, that receive information about your visits and form submissions so we can measure and improve our advertising. Under some U.S. state privacy laws, including California’s, this may count as “sharing” personal information for cross-context behavioral advertising, or as “targeted advertising”. You can opt out at any time, as explained under Your privacy rights.

Service providers we use

  • HubSpot stores form submissions, contact records and call bookings, and sets cookies to connect visits to a contact record.
  • Google Analytics and Google Tag Manager measure site use.
  • Google Ads measures which ads lead to a request.
  • Our hosting provider runs the site and its caching.
  • GeoJS estimates your city from your IP address so the site can show information for your area. The estimate stays in your browser for that visit only and is not stored by us.

If we add other advertising platforms, such as Meta, they would receive the same kind of visit and conversion information. Each provider handles data under its own terms and privacy policy.

Your choices

  • Cookies: block or delete cookies in your browser settings. Blocking them does not stop the site from working.
  • Google Analytics: install Google’s opt-out browser add-on at tools.google.com/dlpage/gaoptout.
  • Email: every marketing email includes an unsubscribe link.
  • Your records: email us to ask what information we hold about you, to correct it or to delete it. We will respond within a reasonable time and may need to confirm your identity first.

Your privacy rights

Depending on where you live, including California and other U.S. states with privacy laws, you may have the right to:

  • know what personal information we collect, use and disclose, and get a copy of it;
  • correct inaccurate information;
  • have your information deleted;
  • opt out of the “sale” or “sharing” of your personal information and of targeted advertising;
  • not be treated differently for using any of these rights.

To use these rights, email contact@companian.health with the subject “Privacy request”. To opt out of sharing for advertising, you can also block cookies in your browser or use the browser controls offered by Google and other ad platforms. We will confirm your request, may need to verify your identity, and will respond within the time the law requires. You may use an authorized agent to make a request on your behalf.

How long we keep information

We keep contact and practice details for as long as we are in conversation with you or working together, and then for as long as we need them for our records or the law requires. Analytics data is kept for 14 months.

Security

The site is served only over encrypted HTTPS connections. We use reputable providers, limit access to the people who need the information, and do not ask for or accept patient health information through the website. No website or online service can promise complete security.

Children

This website is for healthcare professionals. It is not directed to children, and we do not knowingly collect information from anyone under 16.

Changes to this policy

We may update this policy. The effective date at the top shows when it last changed.
