Urologic conditions we follow between visits
What the Care Advocate asks about for each, and where the answers go.
Prostate cancer on surveillance or hormone therapy
PSA draws and imaging follow your urologist’s interval. The Care Advocate reminds the patient, confirms each test and each androgen deprivation injection visit.
BPH with chronic urinary retention
Your plan’s voiding and catheter questions are asked and recorded monthly. Supply or home-health questions go to your staff, who handle orders.
Neurogenic bladder
Patients read their voiding or catheterization diary by phone or video. Entries outside your plan’s limits go to your office.
Recurrent kidney stones
Your plan’s stone-prevention questions, such as daily fluid intake, are asked and recorded. Ordered urine studies and imaging are confirmed with the patient.
Blood in the urine, fever or a blocked catheter goes to your clinicians under your escalation rules. Care Advocates do not interpret results or change catheter instructions.

PCM, CCM or PIN: which code fits each patient
Count the conditions your practice manages, then check each program’s CMS test.

| Program and codes | Fits when | Time in the calendar month | 2026 payment, national |
|---|---|---|---|
| PCM · 99426, 99427 | One serious, high-risk chronic condition expected to last at least 3 months, such as prostate cancer your practice manages | At least 30 minutes of clinical staff time; 99427 adds each further 30 | $67.80 national |
| CCM · 99490, 99439 | Two or more chronic conditions expected to last at least 12 months that place the patient at significant risk, both managed by your practice | 20 minutes for 99490; 99439 adds each further 20 | $66.13 and $50.44 national |
| PIN · G0023, G0024 | One serious, high-risk condition expected to last at least 3 months with risk of hospitalization, functional decline or death | 60 minutes for G0023; G0024 adds each further 30 | $87.18 and $54.44 national |
BPH plus stones: when CCM fits
A patient with BPH, chronic retention and recurrent kidney stones may have two qualifying conditions you manage. If your urologist judges both meet the CMS test, CCM applies. CCM requirements.
Advanced prostate or bladder cancer: PIN
PIN needs an initiating E/M visit by your practitioner (not 99211) and staff trained on that condition. It can run alongside PCM or CCM if no minute counts twice. See PIN codes G0023 and G0024.
Catheter patients under home health
CMS bars CCM in the same period as home health or hospice supervision (G0181, G0182). Check catheter patients’ months before billing CCM.
How a urology month runs with Companian
Your plan sets the test calendar. The Care Advocate keeps patients on it.
You send the list
Your staff send the list, by referral form or EHR access, with PCM, CCM or PIN marked for each patient.
Approval, then consent
After your one-time outreach approval, the Care Advocate explains the program and the right to stop, then documents verbal consent.
Calls timed to the next test
Each month’s call is timed around the next PSA draw, scan, injection visit or diary review in your plan.
Your team signs off the file
The file lists minutes by activity and each consent date. Claims go out only after your team approves it.

What stays with your urologists
The Care Advocate asks, records, confirms and routes. Every clinical step is yours.
| Task | Your urology team | Companian Care Advocate |
|---|---|---|
| PSA and imaging results | Reads them, explains them and decides what happens next | Reminds the patient and confirms the test took place |
| Hormone therapy | Sets the drug, dose and schedule | Confirms the next injection visit with the patient |
| Catheter care | Writes the instructions, places supply orders and contacts home health | Records supply and delivery questions and passes them on |
| Care plan | Writes it and updates it | Asks its questions each month and records the answers |
| Medicare claim | Bills under its own NPI through its Medicare contractor | Prepares the monthly file and never bills Medicare |

Chronic care management for urology practices: FAQ

Can we bill CCM if primary care already bills it?
Not for the same patient in the same calendar month: CMS allows one CCM biller per patient per month. In that case, compare PCM for the urologic condition.
Does BPH qualify on its own?
Not by diagnosis alone. Your urologist judges whether BPH with retention or other complications is serious and high-risk enough for PCM, or one of two qualifying conditions for CCM. Our explainer on what chronic care management is sets out the eligibility basics.
Can our urologist’s own time be billed?
Yes, under PCM. 99424 covers your urologist’s first 30 personal minutes in a month ($87.51 national in 2026). 99425 covers each additional 30. Care Advocate time uses 99426 and 99427, never the same minute.
What do patients pay?
Medicare Part B cost-sharing: the deductible and 20% coinsurance, which supplemental coverage or Medicaid may pay. Patients hear this before they agree to take part.
Rates & disclosures
National amounts are 2026 Medicare Physician Fee Schedule non-facility payments at the non-QP conversion factor of $33.4009, computed from CMS RVUs. Your amount depends on your Medicare payment locality; see it in the CCM/PCM revenue calculator. Actual payment depends on participation status, place of service and cost-sharing.
Companian supplies Care Advocates and documentation to physician practices nationwide. Companian is not a medical practice and does not bill Medicare. The billing practitioner keeps the care plan, every clinical decision and the claim. Patients pay the usual Medicare Part B deductible and coinsurance for these services unless other coverage pays it.
