Oncology CCM and PCM eligibility

Which oncology patients fit which program

In chronic care management for oncology practices, eligibility is confirmed at the initiating visit.

Patient groupWhat the Care Advocate covers each monthProgram to consider
Active treatment: chemotherapy, immunotherapy or radiationConfirms infusion, radiation and lab dates; asks your plan’s symptom questionsPIN (G0023, G0024) for one serious, high-risk condition
Survivorship and surveillanceConfirms surveillance scans, labs and visits on your plan’s schedulePCM, if the condition still meets the high-risk definition
Chronic blood cancers on oral therapy, such as CLL, CML or multiple myelomaAsks your plan’s adherence and side-effect questions; confirms lab draws and visitsPCM for one cancer; CCM for two or more chronic conditions
Any group above, when transport, food or caregiving gets in the wayWorks on the social need behind a missed visitCommunity health integration (G0019, G0022)
Your practitioner makes the call.

PIN and PCM need one condition lasting at least 3 months. CCM needs two or more, expected to last at least 12 months. They must put the patient at significant risk of death, acute exacerbation or decompensation, or functional decline. See eligible conditions.

PIN minutes and limits

G0023 needs 60 minutes of navigation in the calendar month; G0024 adds each further 30. CMS allows PIN once per practitioner per month per serious, high-risk condition. Staff need training on that condition. PIN codes G0023 and G0024.

PCM and CCM floors

PCM has a 30-minute monthly floor; use 99424 when your oncologist spends the time personally. CCM starts at 20 minutes of clinical staff time (99490). Only one practitioner can bill CCM per patient per month, so check whether primary care already does. Complex CCM has separate time and care plan rules; our guide to CPT 99487 and 99489 covers them.

Hospice months and stacked programs

CCM cannot be billed in the same period as home health or hospice supervision (G0181, G0182). PIN and CHI can run alongside other care management if no minute counts twice. For conversations about future care wishes, see advance care planning (CPT 99497).

Care Advocate and oncologist roles

What Care Advocates handle and what oncologists keep

Care Advocates never assess side effects, triage or give medical advice.

SituationCare AdvocateYour clinical team
Patient reports fever or a new side effectRecords it and routes it under your escalation rulesAssesses, triages and decides
Missed infusion or scanAsks why and tells your scheduling teamReschedules and decides whether treatment timing changes

Escalation rules come from your care plan.

Care Advocates are registered nurses, nurse practitioners, licensed clinical social workers and care companions. Your billing practitioner provides general supervision under the services agreement; have counsel confirm it.

Oncology care management FAQ

Questions oncology practices ask

Clinician with an older patient in a chronic care management program
What triggers a call to our nurse line?

Whatever your care plan lists as an escalation trigger, plus anything new the patient raises. Your clinicians write the rules; the Care Advocate applies them and does not judge urgency.

Is PIN consent different from CCM consent?

Yes. CCM consent is documented once, before billing. PIN consent comes before or when services start and is renewed yearly. The monthly file shows the consent date. More on consent.

Can navigation happen by phone or video?

Yes. CMS allows PIN and CHI by telecommunications technology. Care Advocates work fully remotely, by phone and video.

Do patients pay for navigation?

Yes. The Part B deductible and coinsurance apply to PIN, PCM and CCM. The Care Advocate explains cost-sharing before consent is recorded.

Who bills Medicare, and how is Companian paid?

Your practice bills under its own NPI and receives the payment. We walk through our fee on the intro call. How cost works.

National amounts are 2026 Medicare Physician Fee Schedule non-facility payments at the non-QP conversion factor of $33.4009, computed from CMS RVUs. Your amount depends on your Medicare payment locality; see it in the CCM/PCM revenue calculator. Actual payment depends on participation status, place of service and cost-sharing.

Companian supplies Care Advocates and documentation to physician practices nationwide. Companian is not a medical practice and does not bill Medicare. The billing practitioner keeps the care plan, every clinical decision and the claim. Patients pay the usual Medicare Part B deductible and coinsurance for these services unless other coverage pays it.
