GI and liver conditions, matched to a program
Your gastroenterologist or hepatologist confirms eligibility at the initiating visit.
| Condition | Covered on the monthly call | Program to consider |
|---|---|---|
| Crohn’s disease and ulcerative colitis | Infusion and lab dates confirmed; the bowel-symptom questions in your plan asked and recorded | PCM when IBD is the one condition you manage |
| Cirrhosis | Liver ultrasound and lab intervals confirmed; new symptoms on your plan’s list routed to your office | PCM, or PIN (G0023, G0024) once decompensation brings significant risk of hospitalization |
| Chronic hepatitis B or C | Lab follow-up dates confirmed; the questions your plan sets asked each month | CCM when your hepatology practice also manages the patient’s cirrhosis |
| MASLD | Diet and alcohol questions from your plan asked and recorded; imaging dates confirmed | PCM, where your practitioner judges it serious and high-risk |
| Any of the above, when cost or transport blocks care | Work on the social need behind missed infusions or procedures | Community health integration (G0019, G0022) |
The condition count decides the code. Two or more chronic conditions expected to last 12 months, like cirrhosis plus chronic hepatitis B, meet the CCM test. Eligible conditions list.

What happens between procedures
Your staff share the list, approve outreach and approve the monthly file.
Consent on the first call
After your approval, the Care Advocate explains the program and Part B cost-sharing and records verbal consent.
Surveillance intervals
Your plan sets the intervals: liver ultrasound, labs on a biologic. The Care Advocate confirms each booked appointment was kept.
Procedure and prep dates
Before a colonoscopy or endoscopy, we confirm the dates and your written prep instructions. Prep questions go to your nurse.
Notes and pre-visit summary
Gloria, an AI assistant, drafts call notes and a pre-visit summary for the next GI visit. People make every call.

PCM for IBD or liver disease
PCM needs at least 30 minutes in the calendar month. Your gastroenterologist’s own time and clinical staff time bill under different codes. Patients with two or more qualifying conditions may fit CCM instead; see what chronic care management is.
PIN for decompensated cirrhosis
A patient facing several procedures and transplant evaluation visits may fit PIN. It needs an initiating E/M visit (not 99211), condition-trained navigators and yearly consent renewal. G0023 and G0024 rules.
CHI for cost and transport barriers
G0019 covers the first 60 minutes of CHI work in a month and G0022 each additional 30, after an initiating visit by your practitioner.
Care Advocates confirm. Your GI team decides.
Care Advocates do not interpret results, change medicines or give medical advice.

| Ultrasound or labs due | Confirms the appointment your office booked | Order and read the study |
| Alcohol or diet questions in your plan | Asks them as written and records the answers | Counsel the patient |
Companian’s registered nurses, nurse practitioners, licensed clinical social workers and care companions work fully remotely under your billing practitioner’s general supervision. The services agreement defines it; have your counsel review it.
