Heart failure, AFib and hypertension

Heart conditions your care plan can cover

Each call follows the questions and thresholds your cardiologists write.

Heart failure

Your plan sets the daily weight and swelling questions. The Care Advocate asks them and routes weights outside your thresholds to your clinicians.

Atrial fibrillation

Missed anticoagulant doses, bruising or bleeding, and new prescriptions from other doctors are recorded and routed under your plan’s escalation rules.

Coronary artery disease

Cardiac rehab, echo, stress test and device clinic visits are confirmed. Your plan’s chest symptom questions are asked word for word.

Hypertension

Home-cuff readings are logged on each call for your chart. Readings outside your targets go to your clinicians.

Monthly cardiology CCM workflow

How chronic care management for cardiology practices runs

Your staff handles three steps. The Care Advocate handles calls and paperwork.

01

Pick the patients

Share eligible Medicare patients through EHR access or a short referral form, starting with the diagnoses you pick.

02

Approve outreach once

The Care Advocate calls each approved patient, explains the program and its Part B cost-sharing, and documents verbal consent. Staff new to the program can start with our explainer, what is chronic care management.

03

Calls follow your plan

The same Care Advocate calls monthly, asks your plan’s questions, records readings and confirms rehab, imaging and device visits.

04

Approve one file

Your billing team signs off on each patient’s program, code family, month, minutes by activity, activities and consent date.

CCM with RPM and TCM

RPM, TCM and hospice rules for cardiology CCM

Three CMS rules decide what else bills in a care management month.

Medication list after hospital discharge with dose changes and a completed follow-up (sample data)

Remote patient monitoring alongside CCM

CMS lets CCM run with RPM or remote therapeutic monitoring, not both. For 2026, 99445 covers 2 to 15 device days, 99454 covers 16 to 30, and 99470 is a new first-10-minute RPM management code. Each minute counts once. See CCM vs RPM.

Transitional care management months

CMS MLN909188 allows CCM during the 30-day TCM period. Your practitioner does the face-to-face visit: within 14 days for 99495 ($220.11 national) or 7 days for 99496 ($298.60 national).

Home health and hospice

CCM can’t share a period with home health or hospice supervision (G0181, G0182). When an end-stage heart failure patient enters hospice, hold that month’s CCM claim.

Cardiology CCM

Chronic care management for cardiology

For cardiology, check:

Conditions

CMS examples include atrial fibrillation, cardiovascular disease and hypertension (MLN909188).

Existing CCM

Ask which conditions primary care’s plan addresses. If it includes hypertension, cardiology PCM must address a different condition (CMS CCM FAQ).

For a patient also on remote monitoring, CMS allows remote physiologic monitoring (RPM) or remote therapeutic monitoring (RTM), one or the other, alongside CCM or PCM, as long as no time is counted twice (MLN909188; CY2024 final rule). Companian does not offer RPM.

Cardiology CCM FAQ

Questions cardiology practices ask

Clinician with an older patient in a chronic care management program
How many minutes does a heart failure patient need each month?

At least 30 clinical staff minutes for PCM (99426) or 20 for CCM (99490). Each further 30 adds 99427; each further 20 adds 99439. The monthly file shows which threshold was met. Complex CCM uses its own thresholds; see complex chronic care management codes.

What triggers a call back to our office?

Whatever your care plan lists: weight gain past your threshold, blood pressure outside your targets, a new symptom or a medicine started elsewhere. The Care Advocate records and routes it to your clinicians.

Which patients should we start with?

The ones you choose, such as heart failure patients who see no other cardiologist, or AFib patients on anticoagulation. See eligible conditions for the CCM definition.

Who makes the calls?

Remote registered nurses, nurse practitioners, licensed clinical social workers and care companions run our chronic care management for cardiology practices. Gloria, our AI assistant, drafts documentation and pre-visit summaries; people make every call. Care Advocates record and route; they never adjust medicine.

Who supervises the Care Advocates?

Your billing practitioner, under general supervision as CMS allows. The services agreement sets the arrangement; confirm it with your counsel.

How is Companian paid?

Your practice bills Medicare under its own license and receives the payment. We walk through our fee on the intro call; see CCM cost for how the numbers work.

Companian supplies Care Advocates and documentation to physician practices nationwide. Companian is not a medical practice and does not bill Medicare. The billing practitioner keeps the care plan, every clinical decision and the claim. Patients pay the usual Medicare Part B deductible and coinsurance for these services unless other coverage pays it.
