Which program fits each neurological condition
Your neurologist picks the program at the initiating visit.
| Condition | Monthly questions and confirmations | Program to consider |
|---|---|---|
| Parkinson’s disease | Dose-timing and fall questions from your plan; PT, OT and speech therapy attendance confirmed | PCM when it is the one condition you manage |
| Multiple sclerosis | New-symptom questions; infusion and MRI dates confirmed | PCM; CCM if you also manage a second chronic condition |
| Epilepsy | Seizure diary read back and recorded; EEG dates confirmed | PCM for the seizure disorder you follow |
| Dementia, ALS and other advancing conditions | Function and safety questions your plan sets; visits confirmed | PIN (G0023, G0024) when risk of hospitalization or nursing home placement is significant |
| Life after a stroke | Therapy attendance and fall questions; follow-up visits confirmed | CCM if post-stroke care is one of two or more chronic conditions you manage |
In chronic care management for neurology practices, the count of conditions decides the code. PCM covers one serious, high-risk condition expected to last at least 3 months. CCM covers two or more lasting 12 months or longer that carry significant risk. Which conditions qualify.

How a month of neurology follow-up runs
Your staff shares the list, approves outreach and approves the month-end file.
Program and consent
After the initiating visit, your neurologist picks PCM, CCM or PIN. The Care Advocate then calls and records verbal consent. For background on the program, read what chronic care management is.
Condition questions
Your plan’s questions, asked as written: dose timing for Parkinson’s, seizures for epilepsy, new MS symptoms, falls after stroke.
Appointments confirmed
MRI, EEG, infusion and therapy appointments your office booked are confirmed in advance. Missed ones go to your front desk.

|---|---|---|

Medicare pays by payment locality, set by state and county: 109 localities nationwide. For CPT 99490 the 2026 amount runs from $60.71 in Arkansas to $83.76 in Alaska.
PCM for the condition you follow
PCM is not paid below 30 minutes in a calendar month. Staff time goes under 99426 and 99427; your neurologist’s personal time goes under 99424. PCM CPT codes.
PIN for advancing disease
PIN starts after an initiating E/M visit by your practitioner. Navigating staff must be trained on the condition, and consent is renewed yearly.
CCM and the one-practitioner rule
CCM requires a comprehensive care plan, 24/7 access to the care team for urgent needs and a designated team member for routine appointments. See CMS booklet MLN909188. Only one practitioner can bill it per patient each month.
What Care Advocates record and neurologists decide
Care Advocates never adjust a dose, read a scan or give medical advice.
| Situation | Care Advocate | Your clinicians |
|---|---|---|
| Seizure reported in the diary | Records date, count and description, then routes it under your escalation rules | Review the pattern and decide on medication |
| Missed Parkinson’s dose or a change in timing | Asks your plan’s timing questions and records answers | Adjust timing or doses |
| A fall at home | Asks your plan’s fall questions and reports falls to your office | Assess the patient and order therapy or imaging |
| Missed MRI, EEG or infusion | Asks the reason and tells your front desk | Reschedule and decide next steps |
The remote care team: registered nurses, nurse practitioners, licensed clinical social workers and care companions. They work under your billing practitioner’s general supervision, per the services agreement; have counsel confirm it. Behavioral health needs fall under a separate code; see general behavioral health integration (CPT 99484).

Questions neurology practices ask

Can our nurse practitioners or PAs bill CCM?
Yes. For CCM, CMS lists physicians (MD or DO), nurse practitioners, physician assistants, clinical nurse specialists and certified nurse-midwives as billing practitioners.
Is PIN limited per month?
Yes. CMS allows it once per practitioner per month for each serious, high-risk condition. Time past the first 60 minutes adds G0024 in 30-minute steps.
Can CCM continue once a patient with ALS or dementia enters hospice?
Not in the same period. CMS bars CCM alongside home health or hospice supervision (G0181, G0182). Your billers should check those months before claims go out.
Do patients share the cost?
Yes. Medicare Part B cost-sharing applies to PCM, CCM and PIN, and patients hear about it before they consent.
How is Companian paid if our practice bills?
Through a fee we walk through on the intro call. Your practice bills Medicare under its own NPI and keeps the payment. See how cost works.
Rates & disclosures
National amounts are 2026 Medicare Physician Fee Schedule non-facility payments at the non-QP conversion factor of $33.4009, computed from CMS RVUs. Your amount depends on your Medicare payment locality; see it in the CCM/PCM revenue calculator. Actual payment depends on participation status, place of service and cost-sharing.
Companian supplies Care Advocates and documentation to physician practices nationwide. Companian is not a medical practice and does not bill Medicare. The billing practitioner keeps the care plan, every clinical decision and the claim. Patients pay the usual Medicare Part B deductible and coinsurance for these services unless other coverage pays it.
